Please email us or call to check your insurance benefits first! We will not schedule an evaluation until all information is collected including insurance, payment plan, therapy packets, PCP/MD orders and required paperwork is signed.
Our preference is that you email us at [email protected] for all therapies except ABA to receive a link to fill out packet's through our EHR system but if you prefer handwriting you can print these packets and send in: ABA email to get EHR packet link to: [email protected]
